Indemnity

I hereby ask the leaders to act in my place if I am not on the premises while my child is at Christ Church Tygerberg.  I hereby indemnify the event leaders against any claim of accidental injury to my child or loss or damage to his/her belongings. I also agree that in a medical emergency, when unable to contact me, the leader in charge may make a decision on my behalf.  I agree to take responsisility for any costs as a result. I further undertake to ensure that my child/children gets collected by me or an adult known to me at the end of the event/program.
