Indemnity form: I request that the leader / s in charge act IN LOCO PARENTIS during this event. I understand that in the case of any illness or accident where medical attention is required, the cost thereof will be my liability. Photographs taken of my child (you are NOT required to tick this). These photos will not be on social media. I agree to drop off my child and pick them up at the times specified by our kids ministry on our church website, on Sunday mornings or at any of our kids programmes.
